Health Care Transition Skill Building: Tools for Students, Families and the IEP Team

Learning and trying out life skills is an important step for students as they plan to move into adulthood. For many students with special health care needs and/or disabilities, planning for that transition can include developing skills needed to manage their own healthcare. PEATC and Got Transition have created a health care transition checklist to help students, families, and the IEP team plan for this specific area of transition to adulthood.
