  • National Academy of Elder Law Attorneys – Searchable database for Elder Law Attorneys.

  • Overview Video of Supported Decision Making – This is presented by Michael Kendrick, Director of Supported Decision-Making Initiatives, Center for Public Representation.

  • Power of Attorney PDFs – This website provides POA documents by state and by type. Make sure to scroll down the entire page to read the procedures and requirements for signature that are near the bottom of the page.

  • Supported Decision-Making Guide – This guide is put together by the Disability Rights Maine.  Some things are Maine-specific, but most of the guide, including the checklist, is universal.

  • Virginia Microboard Association – A microboard is a formalized “circle of support,” a non-profit corporation consisting of a small group of individuals who assist an individual with disabilities in creating and implementing a life plan and making decisions.
