- Charting the Course for the Future – A Transition Toolkit – While this toolkit was developed by the Colorado Department of Education and some of the resources and information are specific to Colorado, there are also some great tools within this toolkit that can be useful for families in VA.
- Facilitating Transitions for Students with Disabilities from High School to Post-School Settings – This online training module provides information for counselors and other education professionals to assist high school students with disabilities in the transition from the school environment to a post-school setting. While it was designed for professionals, it includes great information for parents as well.
- Transition Services – Next Steps NH (nextsteps-nh.org) – This website provides easy-to-understand information about the transition process as well as various transition toolkits and examples of transition plans. While some of the information is New Hampshire specific, most of the information can be used by anyone.
- Life Skills Inventory – This independent living skills assessment tool covers 15 components of independent living to identify areas of growth potential and IEP goals.
